Evaluation
and Consultation Fees
Dr. Kelly conducts all neuropsychological
evaluations directly, including assessment of intelligence, academic
skills, social-emotional functioning, attentional abilities, executive
functions, language skills, visual-spatial skills, and adaptive skills. The
evaluation includes time for reviewing records, scoring tests and writing a
comprehensive report.
Families are responsible for payment
at time of service. All other
meetings including office visits, school meetings, direct consultation, and
IEP attendance are billed at the rate of $125 per hour due at the time of
service. Checks and credit cards are readily accepted. An itemized invoice is provided at the
end of an evaluation process upon request. Missed appointments and
appointments not cancelled with 24 hours advanced notice are billed at the
full hourly rate. Dr. Kelly does not accept insurance directly; however, an
itemized invoice that parents can submit for reimbursement is provided.
